In dealing with ongoing or significant unsatisfactory performance or behavioural issues, managers should clearly identify, assess, and analyse the problem before speaking with the employee on a formal basis. This form should be utilised by managers/supervisors to adequately prepare for the commencement of a Performance Improvement Plan (PIP) process and be completed prior to the development of a PIP form or an initial meeting with the underperforming employee. It is recommended that employers use this checklist in conjunction with the other PIP resources available.
