This program will provide the practical steps staff need to undertake when investigating workplace incidents.  This course will cover the processes involved in systematically investigating incidents, and identifying both direct and root causes as well as understanding internal/external notification processes to meet compliance requirements.  Understanding and identifying the real cause of an incident is paramount in minimising workplace incidents. 

At the completion of the course participants should be able to: 

  • Outline the system for recording and reporting incidents. 
  • Understand the stages of incident investigation and learn to identify key information when investigating incidents. 
  • Outline how to correctly conduct an incident investigation, identifying both the direct and root causes of the incident.
  • Understand how to learn from incidents to apply safety systems improvements
