This Sample: Interview Scoring Matrix Form for all Interviewed Applicants can be used to collate scores applied by interview panel members for all applicants for a particular role following the conclusion of all interviews.

A sample Individual Interview Assessment Form is also available which can be used to record individual applicant scores by each interview panel member during or shortly after the interview takes place.

This resource can be used to collate those scores during a post-interview meeting or by the overseer/HR department when all forms are submitted. It should be noted that there may be additional selection methods, other than the interview. If this is the case, this form can be adapted to also include the collation of results from other selection methods, such as Psychometric Testing, a second interview, work sample tests etc.
