This form may be used to record a history of incidents, illness, injury and disease in the workplace.  A record of workplace injuries or illnesses sustained by workers, regardless of whether there has been a worker’s compensation claim, must be recorded and maintained.  Information in this form is of confidential nature and must be kept secure and private and only accessible by authorised persons of the organisation.

This document can also assist the organisation to monitor data and trends related to workplace injuries and assist to implement strategies for continuous improvement in the workplace and to reduce the risk of further injury.
