An incident at the workplace, or offsite involving your workers, needs to be dealt with quickly and calmly. The Incident Report Form should be completed as soon as possible following an incident.
If it is not possible for the person involved in the incident to complete the form, it may be completed by a supervisor (or other worker) on their behalf. It is important to record incidents to assist with hazard identification and risk management, to identify any health and safety incident trends before they escalate and to ensure details are recorded for workers’ compensation eligible incidents and notifiable incidents.
Workers should be provided training during induction to ensure they are aware of the requirement to report incidents, where to locate the form and how to correctly complete the incident report form.
