The Performance Development Review, sometimes referred to as a performance appraisal or evaluation, evaluates the overall performance and development of employees during the performance management cycle, or defined performance period, and applies a performance rating, as well as assesses past, current and future performance development needs.
The primary objective of the Performance Development Review is to support the employee in achieving their highest level of performance by creating awareness as to their strengths and areas that require development.
This Performance Development Review Form has been predominantly written addressing the employee directly. This will help to ensure that the employee is the main focus of the plan and takes accountability for the achievement of their objectives and their development.
NB: To assist in the completion of this form examples have been included in some sections. These can be deleted to enable the completion of the form. Also, an example rating scale has been included. Organisations should insert the rating scale appropriate to meet their performance management needs. Examples of alternative rating scales can be referred to in the document ‘Sample Rating Scales’.
