This Workplace Flexibility – Employer Consideration Form is for completion by the employer upon receipt of a Workplace Flexibility Request Form from an employee. The Request Form should be attached.

This form enables an organisation to record the outcome of the request, including consideration of the Request Form and any other information requested by the employer and supplied by the employee to assist with a determination.

Consideration of the request and the subsequent decision should be in line with the organisation’s Flexible Working Policy.

The workflow below assists with the steps required to be undertaken.  On completion, a copy of the form should be placed in the employee’s personnel file and confirmation of the outcome provided to the employee in writing.

NB If the employee is applying as an eligible employee under the National Employment Standards, this process must be completed within 21 days.
